Job Description
The founders of Day City Hospice took the lessons learned from years of experience in corporate America to create a patient-centered hospice that maximizes resources at bedside while being operated locally. Our passion is to provide patients and their families with hospice care that is rooted in the communities we serve. When end-of-life care is necessary, patients and their families are in their most vulnerable time. Our Ohio-based management team, through our local clinicians, can respond to the needs of our patients in a timely and appropriate manner because we are local. We are also passionate about providing our employees the right tools to do their jobs effectively and provide the best hospice care in the Dayton, OH area. We don't require high caseloads or unmanageable quotas, which gives our team more time with each of their patients. Our top priority is our patients and the people serving them.
Schedule:
Monday-Friday 8a-5p with two holidays on call each yearLocation:

Assume primary responsibility for patient and family caseload, including the assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care Provide regular visits to re-assess the patient's needs and update the plan of care Perform clinical pronouncement of death and provide support to the family Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelinesQualifications:
