Job description Overview Join our compassionate healthcare team as a Hospice Social Worker, where you will play a vital role in providing emotional support, counseling, and case management to patients and their families during one of life's most sensitive times. This dynamic position offers the opportunity to make a meaningful difference by delivering holistic care that addresses social, emotional, and psychological needs. You will collaborate closely with interdisciplinary teams to ensure patients receive comprehensive, person-centered hospice services that promote dignity, comfort, and quality of life. Duties Conduct thorough patient assessments to evaluate social, emotional, and psychological needs within hospice care settings. Provide individual and group counseling sessions utilizing clinical techniques such as behavioral therapy, motivational interviewing, and psychotherapy tailored to patients' conditions. Develop personalized care plans that incorporate child & family counseling, addiction counseling, dementia care, PTSD care, and working with individuals with disabilities or developmental disorders. Coordinate with healthcare teams to facilitate intake processes, medical documentation, and ensure compliance with HIPAA regulations for patient privacy and confidentiality. Assist families in understanding medical terminology, ICD coding (International Classification of Diseases), CPT coding (Current Procedural Terminology), and navigating complex medical records and documentation systems like EMR (Electronic Medical Records). Offer crisis intervention and behavioral health support for patients experiencing acute distress or behavioral challenges related to terminal illness or mental health conditions. Manage casework responsibilities including discharge planning, home & community care coordination, assisted living arrangements, and post-acute care transitions. Experience Proven experience in hospice care or palliative settings with a focus on patient care and assessment. Strong background in social work or mental health counseling with familiarity in clinical research and data collection methods. Experience working in inpatient facilities such as hospitals or nursing homes; home & community-based care experience is highly valued. Knowledge of medical terminology, ICD-10/ICD-9 coding systems, CPT coding, and medical documentation standards aligned with HIPAA compliance. Skilled in working with diverse populations including seniors (geriatrics), individuals with autism or developmental disabilities, Alzheimer's disease patients, and those requiring specialized dementia or PTSD care. Ability to utilize EMR systems effectively for documentation and case management purposes. Excellent crisis management skills combined with the ability to perform diagnostic evaluations and mental health assessments using cognitive behavioral therapy techniques. Experience in outpatient or clinic environments involving individual/group therapy sessions focused on behavioral health or psychotherapy modalities such as applied behavior analysis or behavioral therapy. $60,000.00 - $70,000.00 per year
