Job Description
Job Summary We are seeking a dedicated and compassionate Social Worker (MSW, LMSW, LCSW) to join our team. The Hospice Social Worker is responsible for providing psychosocial assessments, emotional support, counseling, resource coordination, and advocacy for hospice patients and their families. This role works collaboratively with the interdisciplinary team to enhance quality of life, support coping, and ensure patients and families feel cared for every step of the way. Duties Complete psychosocial assessments to identify emotional, social, financial, environmental, and caregiver needs. Develop and implement individualized care plans based on patient and family goals and needs. Provide emotional support, counseling, and crisis intervention to patients and families coping with terminal illness, grief, anticipatory loss, and end-of-life transitions. Assist patients and caregivers with advance care planning, community resources, financial assistance programs, and long-term care planning. Collaborate closely with the interdisciplinary team including nurses, chaplains, aides, and providers to ensure holistic, patient-centered care. Provide education and support regarding hospice services and the end-of-life process. Facilitate family meetings and support communication between patients, caregivers, and the care team. Support bereavement needs and collaborate with the bereavement team as appropriate. Maintain timely, accurate, and compliant documentation in accordance with hospice regulations and organizational policies. Participate in interdisciplinary team meetings and contribute to care planning discussions. Advocate for patient dignity, comfort, and quality of life. Requirements Master's Degree in Social Work (MSW) required. Current social work licensure in the state of Illinois (LSW/LCSW preferred, if applicable). Previous hospice, palliative care, healthcare, or counseling experience preferred. Strong communication, empathy, and relationship-building skills. Ability to navigate difficult conversations with compassion and professionalism. Excellent organizational and documentation skills.
