Care Initiatives is currently seeking a PRN Visit Nurse to provide patient care in Council Bluffs, Avoca, Dunlap, and surrounding communities. What it takes to be a RN Hospice Visit Nurse Collaborate with the physician to oversee and implement personalized care plans, ensuring the active involvement of interdisciplinary teams to achieve the optimal outcomes for each patient Provide consistent, high-quality patient care to ensure an exceptional and seamless patient experience Perform initial and comprehensive assessments on all patients in accordance with policies and procedures Provides high-quality patient care while ensuring clear, concise, and accurate clinical documentation Assesses the need for plan of care updates while keeping the patient and family involved in care planning decisions Why do RN's choose Care Initiatives Hospice? Hospice Visit Nurse:
RN with a minimum of two years of recent clinical nursing experience. Current, valid CPR certification. Demonstrated patience and a supportive and sensitive approach to providing patient, family, and caregiver support. Willingness and ability to provide care in facility and patient home settings.
