Responsibilities:

Weekend 1st On Call (call period is 5pm Friday through 8:30am Monday) Completes an initial assessment of patient and family to determine hospice needs on weekend admissions or level of care change Provides professional nursing care by utilizing all elements of nursing process Counsels the patient and family/caregiver in meeting nursing and related needs Works in cooperation with the family/caregiver and hospice Interdisciplinary Team Members to meet the emotional needs of the patient and family/caregiver Initiates the plan of care and makes necessary revisions as patient status and needs change Initiates appropriate preventive and rehabilitative nursing procedures Administers medications and treatments as prescribed by the physician in the physician's plan of care Assesses and evaluates patient's status by writing and initiating plan of care. Regularly re-evaluates patient and family/caregiver needs. Revises the plan of care as necessary Provides and maintains a safe environment for the patient

Qualifications:

Graduate of an accredited school of nursing Licensed to practice as a RN in the State of South Carolina with a license in good standing Maintain a current CPR certification and valid SC driver's license Minimum two years nursing experience required One to two years of recent acute care experience in an institutional setting, and one to two years of recent experience in home care preferred Bachelor's degree, with one year of hospice care experience preferred Must have reliable transportation for daily travel throughout the service area Must work well within an interdisciplinary team environment as well as function independently at the bedside following policies and procedures and good hospice nursing practices Management experience not required but supervisory skills needed. Responsible for supervising hospice aides Detailed oriented and strong problem solving skills Excellent verbal and written communication skills Lutheran Homes of South Carolina is an equal opportunity employer.
