Duties Deliver personalized patient care by assisting with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patient health through observation of vital signs, medication administration, and symptom tracking while maintaining accurate medical documentation Assist with specialized care needs such as Alzheimer's, dementia, memory care, hospice support, catheter care, and use of medical equipment like Hoyer lifts Maintain strict adherence to HIPAA regulations to protect patient privacy and confidentiality at all times Utilize Electronic Health Record (EHR) systems for documenting medical records, care plans, and progress notes efficiently and accurately Support patients in assisted living or long-term care environments by providing companionship, emotional support, and engaging activities Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe environment for clients Collaborate with healthcare teams to coordinate services using EMR systems and ensure seamless continuity of care in home health or community settings Requirements Valid CNA or HHA certification with current CPR certification required Proven experience working with individuals with developmental disabilities, Alzheimer's disease, dementia or in hospice care preferred Strong knowledge of medical terminology and procedures related to patient observation and medication administration Experience operating medical equipment such as Hoyer lifts and performing catheter care is highly desirable Ability to work independently in live-in or flexible schedules while maintaining high standards of patient safety and comfort Excellent communication skills to effectively interact with clients, families, and healthcare providers Demonstrated ability to handle sensitive situations with compassion while adhering to HIPAA guidelines Prior experience in home & community care or assisted living environments is advantageous Join us in making a meaningful difference!
