Be a trusted helping hand in the moments that matter most. As a Home Health Concierge, you'll provide personalized, non-clinical support to participants—often one-on-one—in their homes or in care settings such as assisted living or skilled nursing facilities. Guided by the interdisciplinary team, you'll focus on enhancing comfort, independence, and overall well-being through meaningful, participant-centered support. • $18.33 / hour Reporting To Tami Loyd Key Responsibilities Provide Hands-On Care
• Assist with personal care (ADLS) like bathing, grooming, dressing, and toileting. Support Mealtime
• Help participants enjoy their meals and stay hydrated for good health. Keep Things Moving
• Support mobility, including turning, positioning, and safe transfers. Perform Vital Checks
• Measure and record vital signs, weight, and intake/output. Maintain Comfort & Care
• Keep rooms tidy, cozy, and well-stocked with essentials. Provide Responsive Support
• Answer call lights quickly and respond with kindness and compassion. Monitor with Watchful Eyes
• Report any changes in conditions to the leadership ASAP. Demonstrate Respect & Professionalism
• Maintain confidentiality while delivering top-notch care.

If selected for the position you will receive a full job description. Skills, Knowledge & Expertise At least one year of experience working with frail elderly or long-term care populations. High School diploma or GED, required Reliable transportation and a valid Michigan Driver's License. CPR certification or willing and able to obtain as offered by LifeCircles.
