Job Description

Referral Liaison - Intake Department, Harbor Hospice Full Time, 40 hours per week, 8am to 5pm - no weekends/holidays
SUMMARY OF JOB RESPONSIBILITIES
The Referral Liaison assures smooth operations of the admissions/intake functions of the agency for Hospice services. This role is responsible for building, maintaining and growing referral partnerships while ensuring a seamless referral process from the first point of contact through admission or service initiation. This position represents the organization professionally and acts as a key driver of referral volume and patient access.
ESSENTIAL FUNCTIONS AND DUTIES
(other duties may be assigned) Serves as primary point of contact for incoming referrals and inquiries regarding programs and services. Maintains and updates referral log, post daily Hospice census and enters daily admissions for disciplines to be assigned. Coordinates administrative duties to support program staff and hospice/palliative care operations. Responsible for Information Systems input (Office 365, EMR, Email, secure messaging) Sending insurance information to billing department timely to ensure smooth transition of care Responsible for setting up transportation of Harbor Hospice patients utilizing contracted agencies and/or area ambulance services Schedules Intake RN with prospective patients and clinical/intake staff. Ability to secure verbal certification of terminal illness from patient's PCP or other certifying physician. Appropriately request medical documentation from referral sources and upload into medical record. Assists Intake RNs with DME, pharmacy, etc. as needed. Initiates traveling contracts to ensure coordination of care when patients are traveling in and out of service area.
