Assisted Living Operations | Healthcare Relationship Management | Community Partnerships About Us We are a rapidly growing physician group specializing in the care of seniors living in Assisted Living Communities, Skilled Nursing Facilities, Independent Living Communities, and other post-acute settings. Our providers deliver highly responsive, relationship-driven medical care while partnering closely with facility leadership to improve patient outcomes, enhance the resident and family experience, reduce unnecessary hospitalizations, and create seamless continuity of care. As we continue our rapid growth, we are seeking an experienced Assisted Living leader to oversee a portfolio of communities throughout the region. Position Overview This is a leadership position for someone who understands Assisted Living operations and enjoys building relationships. The Regional Director of Clinical Partnerships & Patient Experience serves as the operational liaison between the Medical Group, facility leadership, providers, residents, families, hospitals, and Skilled Nursing Facilities.
Your responsibility is simple:
Make sure every community loves working with us. You will become the primary point of contact for Executive Directors, Wellness Directors, providers, residents, and families while ensuring an exceptional experience throughout the entire continuum of care. What You'll Own Community Relationships Build strong relationships with Executive Directors, Wellness Directors, Resident Care Directors, and community leadership. Conduct regular rounds throughout assigned Assisted Living Communities. Ensure leadership feels supported, heard, and confident in the Medical Group's services. Identify opportunities to improve communication, workflow, and service. Proactively resolve issues before they become larger concerns. Patient & Family Experience Build relationships with residents and their families. Serve as a trusted resource for families navigating healthcare decisions. Ensure concerns are addressed promptly. Facilitate communication between providers and families when appropriate. Deliver an exceptional customer service experience throughout the patient's journey. Hospitalization & Continuity of Care Hospitalizations represent one of the most important opportunities to support patients and families.
You will:
Track every resident hospitalization across your assigned communities. Maintain communication with families throughout the hospital stay. Coordinate with hospitals, case managers, social workers, and providers. Help ensure smooth transitions of care following hospitalization. Assist families in understanding available post-acute care options while respecting patient choice and facility processes. Monitor outcomes and identify opportunities to improve continuity of care. Skilled Nursing Partnership Development Regularly round at Skilled Nursing Facilities served by the Medical Group to: Maintain relationships with facility leadership. Monitor patient experience. Identify residents preparing for discharge back into the community. Evaluate potential Assisted Living placement opportunities. Coordinate with Assisted Living communities to facilitate successful transitions when appropriate. Provider Operations Support provider success by: Identifying operational challenges. Improving communication between providers and communities. Monitoring responsiveness and follow-up. Assisting with onboarding new communities. Ensuring consistency across all assigned locations. Key Performance Indicators (KPIs) Success in this role will be measured by more than activity—it will be measured by outcomes.

$70,000.00 - $80,000.00 per year
