Job Description

Day Activities Facilitator —
Full Time Location:
Salisbury, MD, US Salary Range:
$40,000-$55,000 annually
Schedule:
4‑day, 36-hour workweek (Monday-Thursday!) About the Role HealthPort is seeking a dedicated and creative Day Activities Facilitator to design and lead specialized programs that enhance clients' community involvement and independent living skills. In this full‑time position, you will facilitate group activities, provide support to care teams, ensure documentation accuracy, and help clients thrive in a supportive and structured environment. Key Responsibilities Develop and facilitate engaging day program groups that promote independent living skills and meaningful community participation. Track and maintain accurate records of client attendance and participation. Provide timely updates to Community Support Teams regarding client progress. Enter and manage service information using electronic health records (EHR), ensuring all documentation is complete and compliant. Uphold confidentiality standards while following
HIPAA, CARF
accreditation requirements, and Maryland State Regulations. Transport clients to and from program activities as needed. Perform additional duties as assigned by the supervisor. Qualifications High School Diploma or equivalent Experience working with individuals with behavioral health issues. CPR and First Aid certification preferred. Valid full driver's license with a safe driving record. Experience using an Electronic Health Record (EHR) system. Why Join HealthPort?
