Job Description

Discharge Assistant Position Summary:
The Discharge Assistant supports the Care Coordination and Social Work staff to aide in safe and efficient transitions of care. They are responsible for coordination of information and communication and other designated responsibilities for the care management team.
Education:
High School graduate or equivalent.
Training and Experience:
A minimum of one-year experience in healthcare setting.
