5 Description American Traveler is seeking an experienced RN Care Coordinator for a 13-week travel assignment requiring at least one year of case management experience in an acute care setting and an active CA RN license. Details Acute care hospital setting with potential assignment to the Emergency Department Role functions within a Care Management/Care Coordination department Patient population spans multiple diagnoses requiring utilization management, discharge planning, and coordination across care teams Caseload includes Medicare patients, observation/1-day stays, readmissions within 30 days, and diagnosis-related criteria cases 8-hour shifts, 8am-4:30pm, weekdays and weekends Requirements Active CA RN license currently in hand Current
BLS/BCLS
certification Minimum 1 year of recent case management experience in an acute care setting Additional Information Responsibilities include coordinating care for assigned patients across the full episode of illness or treatment, collaborating with patients, families, physicians, the multidisciplinary care team, and payors Utilizes the nursing process (assessment, planning, intervention, evaluation) to facilitate advanced clinical care coordination Evaluates and analyzes quality and utilization cases, including admission/discharge criteria, outlier reviews, and readmission patterns Initiates appropriate clinical pathways based on admission diagnosis and serves as a resource for utilization management Advocates for patients and families, maintaining availability as a communication resource among providers until discharge or transfer W-2 employment status with your staffing agency is required for placement at this facility
