1 Description American Traveler is hiring a Director of Care Coordination with previous acute care leadership experience for a day shift assignment requiring a CA RN license. Details Acute care hospital setting Day shift, Monday through Friday 13-week assignment Up to 36 hours of call-off available to the facility at no cost during the assignment Requirements Active CA RN license required BLS certification required BSN required, with a master's degree in Nursing, Health Administration, or Business Administration preferred Case Management certification (CCM or ACM) preferred Minimum 5 years of experience in an acute healthcare setting required Minimum 5 years of progressively responsible leadership/administrative experience in Case Management, Care Coordination, Utilization Management, or a closely related function required Demonstrated experience leading interdisciplinary clinical/operational programs, staff, performance improvement, and regulatory or accreditation readiness required Advanced knowledge of case management, care coordination, utilization management, discharge planning, patient progression, and transitions of care in an acute-care environment required Additional Information Lead care coordination, case management, and utilization management functions within an acute-care hospital Oversee interdisciplinary clinical and operational programs, staff performance, and regulatory or accreditation readiness
