Educational Assistance Programs Schedule Details/Additional Information:
Full time Monday through Friday schedule Rotating on call schedule for weekends and holidays What You Will Do Lead daily operations of Care Management and Social Work programs, ensuring appropriate staffing, scheduling, team performance, and continuity of services. Drive program performance improvement through data analysis, benchmarking, root cause identification, and implementation of corrective action plans. Partner with physicians, site leadership, and key stakeholders to identify trends, improve care management outcomes, and execute documented improvement strategies. Serve as a subject matter expert and consultant on complex care management cases, regulatory requirements, utilization management, patient flow, and discharge planning. Oversee departmental budgets, resource allocation, and operational efficiencies while maintaining fiscal accountability. Ensure compliance with CMS, OSHA, accreditation standards, ethical guidelines, and state and federal regulations governing care management practices. Manage all aspects of team development, including recruitment, onboarding, competency assessment, performance management, employee relations, and ongoing professional development. What We Are Looking For Active RN or Social Worker license required in the state of practice. Bachelor's degree in Nursing or Master's degree in Social Work required. Professional case management certification (ACM, CCM, RN-BC, or C-SWHC) required within one year of hire. Minimum 5 years of experience in a clinical care management, social work, or related healthcare setting. At least 1 year of leadership or supervisory experience in a Care Management role. Strong knowledge of care coordination, utilization management, regulatory compliance, and interdisciplinary team collaboration. Master's degree in Nursing Administration, Healthcare Administration, or a related field preferred.
