New York Community Care is seeking a compassionate Care Manager to join our growing Assisted Living Center. The ideal candidate will coordinate client care services, support patients and families, and ensure high-quality home care delivery while maintaining compliance with agency and state guidelines. Responsibilities Coordinate and manage home care services for assigned clients Conduct client assessments and develop individualized care plans Communicate with patients, families, caregivers, and healthcare providers Schedule and oversee caregiver assignments and service hours Monitor client progress and update care plans as needed Ensure documentation is accurate, timely, and compliant Address client concerns and provide ongoing support Maintain knowledge of home care regulations and agency policies Interested candidates are encouraged to apply.
