Must be able to pass a background check. Must agree to follow a drug free workplace policy. Able to manage stress well and communicate effectively. Must complete and pass all required state and agency trainings. Must meet continued education requirements, if required. Job Overview A Community Inclusion Specialist will enable the individuals we support in leading fulfilling lives based upon their personal support needs, by doing everything possible to ensure that the individuals we support are satisfied with the level of support they receive. You will achieve this by; communicating with individuals, having person centered thinking, and by following their personal goal and support plans. Community Inclusion support staff will offer "just enough" support to ensure the individuals succeed and offer additional instruction and education to assist them in maximizing their full potential. It is also important that the individuals you support make as many choices as possible, so they remain in control of their own lives. Responsibilities Provide some direct care and support to individuals with disabilities out in the community. Assist client with finding community calendars and resources to find activities of interest that will be meaningful to them and will assist them with reaching their goals. Be trained on the client's goal and follow specific instructions. Assist with engaging in activities of meaningful interest, that allow the individual to develop status and contribution within their community. Assist the individual with staying connected to their community & friends. Collaborate with the in home team with scheduling. Assisting clients with scheduling transportation when participating in activities. Read and understand care plans to ensure you can support the clients support needs. (ex; special diets, medical and/or behavior supports, ect.). Document each activity. Summarize progress monthly. Document days and times of the activity Skills Must be organized and have excellent record keeping. Understanding of diversity, equity, and inclusion principles Excellent communication and interpersonal skills Ability to collaborate with cross-functional teams Experience in developing and delivering training programs Previous experience in a similar role is preferred, experience factor considered and will be discussed at the interview.
