Job Description
Responsible for coordinating and scheduling community-based programming, including coordinating the employment, in-home and community participation programs. An employee in this position is responsible for scheduling coverage of all community based programming, including staff vacation, scheduled days off and unscheduled days off/call offs. This person will also supervise/oversee any PRN staff within the Community Based Services department. Essential duties and responsibilities Develops client/staff schedules for all community-based programming to ensure the needs of the client individual support plan, as well as the community site needs are met Ensures staff/client coverage of all community-based sites in order to meet contractual needs Communicates with community-based specialists in order to track staff vacation/personal days, as well as unscheduled call offs and sick days Be available to work non-traditional hours, including evenings and weekends to ensure coverage for community sites whenever there is a call-off. Maintains a working knowledge and implements the Pennsylvania regulations regarding Home and Community Based Services along with all Office of Vocational Rehabilitation (OVR), and Office of Developmental Program (ODP) initiatives, bulletins, and regulations Responsible for all Community Based referrals and intakes, as well as placements once clients are admitted into the program Must be familiar with the various funding streams associated with the individuals' receiving services throughout the agency such as the Consolidated Waiver, PFDS Waiver, etc. Reports any incidents or unusual incidents to the point person designated by the agency and/or by calling the Incident Management Hotline Maintain all required employment training set forth by OVR and ODP including ACRE certification Comply with all applicable federal, state, local laws and regulations, policies and procedures and reports any concerns about compliance practices. Performs other related duties as required Supervises PRN staff in the Community Based programs. Responsible for ensuring staff are up to date on training and are keeping documentation records.
Required Education:
Bachelor's degree in Social Work, Human Services, Business Administration, or a related field.Required Experience:
Minimum of 5 years of experience in workforce management, community outreach, or human services. Proven track record of managing community-based programs and initiatives. Experience in developing and implementing strategic plans for workforce development. Demonstrated experience in supervising and leading teams to achieve organizational goals.Required Skills and Abilities:
Strong leadership skills with the ability to motivate and manage a diverse team. Excellent communication skills, both verbal and written, with the ability to engage effectively with community stakeholders. Proficient in data analysis and reporting to evaluate program effectiveness and make informed decisions. Ability to develop and maintain partnerships with community organizations and local businesses. Strong problem-solving skills with the capacity to address challenges in a dynamic environment.
