Personal Care Management (PCM)
Services Location:
Worcester, MA (with travel throughout the service area)
Schedule:
Monday-Friday 8:00am-4:30pm
Position Summary:
The PCM Skills Trainer plays a key role in supporting individuals with disabilities who are seeking services through the MassHealth Personal Care Attendant (PCA) Program . This position involves guiding consumers through the PCA program, conducting in-home assessments, delivering skills training, and ensuring compliance with program requirements. The role requires regular travel within the service area

CAR and a strong commitment to promoting consumer independence and empowerment.
Key Responsibilities:
Conduct orientation sessions for individuals interested in the MassHealth PCA Program. Determine eligibility for PCA services and support consumers through the application and evaluation process. Complete in-home assessments to evaluate a consumer's ability to manage their PCA services. Provide skills training using the approved Medicaid curriculum, covering: Employer responsibilities Hiring and managing PCAs Payroll and paperwork processes Overseeing personal care routines Serve as a liaison and advocate for consumers, connecting them with additional community resources. Troubleshoot and provide support related to consumer programs, including peer support, advocacy, skills training, and information & referral. Maintain accurate service records and submit required documentation within designated timelines.
