Job Description
Job Summary Join our compassionate team as an In-Home Certified Nursing Assistant (CNA), where your dedication and skills will directly enhance the quality of life for individuals in their own homes. In this vital role, you will provide personalized patient care, assist with daily activities, and support health and safety needs in a home setting. Your energy and attention to detail will help clients maintain independence, comfort, and dignity while receiving professional care tailored to their unique needs. Duties Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using tools like Hoyer lifts when necessary Monitor patient health by taking vital signs including blood pressure, pulse, respiration, and temperature; accurately document medical records using Electronic Health Record (EHR) systems Observe patients closely for any changes in condition or behavior, especially in memory care or dementia-related cases such as Alzheimer's care and hospice situations Administer medications following prescribed protocols while adhering to HIPAA regulations to ensure confidentiality and safety Support clients with meal preparation and nutritional needs, including cooking and feeding assistance tailored to dietary restrictions Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe environment to promote overall well-being Collaborate with healthcare teams by documenting medical information accurately and communicating patient status effectively during home & community care visits Requirements Valid Certified Nursing Assistant (CNA) certification with current licensure in good standing Proven experience working in assisted living facilities, nursing homes, or home health environments with a focus on senior care or individuals with disabilities Knowledge of medical terminology, medication administration procedures, catheter care, and hospice or dementia-specific care techniques Ability to operate EMR/EHR systems efficiently for documentation purposes CPR certification required; additional certifications such as HIPAA training preferred Experience working with individuals with developmental disabilities or memory impairments is highly desirable Strong patient observation skills combined with compassionate caregiving attitude and excellent communication abilities Join us in delivering exceptional home health services!
