What you'll do as a

Independent Living Case Manager:

Work Monday through Friday 8am-4pm Provide support to 8-9 individuals both in their home and within the community. Utilize your personal vehicle to offer transportation services. Provide role modeling, training and direct care services encompassing all aspects of daily living Aid individuals in managing state issued benefits and government programs. Foster participation in individuals' social and recreational activities tailored to individual interests. Assist in completing the ISP process for DDS funded individuals. Respond to the emergency needs of individuals. Oversee financial budgets, balance check books, bank deposit, timely payment of bills of persons on assigned caseload. What you'll need to be a

Independent Living Case Manager:

High School Diploma or General Education Degree (GED) required Valid driver's license, access to a motor vehicle, and satisfactory driving record Strong written and verbal communication skills Strong computer and word processing skills 1+ years' experience and/or training working with individuals with disabilities preferred. 1+ years' experience in case management preferred.
