Minimum Qualifications:

Preferred Qualifications:

Post-secondary education or training in healthcare, social work, or a related field. Certification as a Certified Nursing Assistant (CNA) or equivalent. Experience working with individuals with developmental disabilities, mental health conditions, or chronic illnesses. Knowledge of person-centered planning and trauma-informed care practices. Bilingual abilities or experience working with diverse cultural populations.

Responsibilities:

Provide direct, hands-on care and support to individuals in accordance with their personalized care plans. Monitor and document changes in health status, behavior, and overall well-being, reporting concerns to appropriate healthcare providers. Assist with daily living activities such as hygiene, mobility, meal preparation, medication reminders, and transportation. Collaborate with multidisciplinary teams to update care plans and ensure coordinated, holistic care delivery. Promote social engagement and community integration by facilitating participation in recreational and therapeutic activities.
