About the
Role:
The Individual Care Specialist plays a critical role in providing personalized support and care to individuals with diverse needs, ensuring their safety, well-being, and dignity. This position involves developing and implementing individualized care plans that promote independence and enhance quality of life. The specialist collaborates closely with healthcare professionals, families, and community resources to address physical, emotional, and social needs. Through compassionate and attentive care, the role aims to empower individuals to achieve their personal goals and maintain a fulfilling lifestyle. Ultimately, the Individual Care Specialist serves as a trusted advocate and caregiver, fostering a supportive environment tailored to each person's unique circumstances.Minimum Qualifications:
High school diploma or equivalent. Certification in CPR and First Aid. Experience working with individuals requiring personal care or support, preferably in a healthcare or social services setting. Ability to pass background checks and comply with state and federal regulations related to caregiving. Strong communication skills and a compassionate, patient-centered approach.Preferred Qualifications:
Post-secondary education or training in healthcare, social work, or a related field. Certification as a Certified Nursing Assistant (CNA) or equivalent. Experience working with individuals with developmental disabilities, mental health conditions, or chronic illnesses. Knowledge of person-centered planning and trauma-informed care practices. Bilingual abilities or experience working with diverse cultural populations.Responsibilities:
Provide direct, hands-on care and support to individuals in accordance with their personalized care plans. Monitor and document changes in health status, behavior, and overall well-being, reporting concerns to appropriate healthcare providers. Assist with daily living activities such as hygiene, mobility, meal preparation, medication reminders, and transportation. Collaborate with multidisciplinary teams to update care plans and ensure coordinated, holistic care delivery. Promote social engagement and community integration by facilitating participation in recreational and therapeutic activities.Skills:
The Individual Care Specialist utilizes strong interpersonal and communication skills daily to build trust and rapport with individuals and their families. Critical observation and documentation skills are essential for accurately tracking health changes and ensuring timely interventions. Problem-solving and adaptability are frequently applied to respond effectively to evolving care needs and unexpected situations. Empathy and patience are fundamental in delivering compassionate support that respects each individual's dignity and preferences.
