Inpatient Care Manager (RN)- Full Time, 8 AM - 4 PM, Atlantic Health Overlook Medical Center

The Inpatient Care Manager is responsible for providing care coordination and services for selected patient populations across the continuum of illness. This role ensures the effective utilization of healthcare resources and monitors care delivery to support optimal clinical and operational outcomes. The Inpatient Care Manager collaborates closely with the Supervisor or Manager of Case Management and leads interdisciplinary efforts to assess, plan, implement, coordinate, monitor, and evaluate services that maximize patient health and ensure safe, timely transitions of care.

Principal Accountabilities:

Manage, coordinate, and facilitate patient care through comprehensive patient and family assessments, including social determinants of health, and collaboration with the interdisciplinary care team. Conduct daily chart reviews, utilization reviews, and coordination of discharge planning activities, ensuring complete, accurate, and timely clinical documentation in the electronic medical record. Lead and participate in interdisciplinary patient care rounds, identifying and resolving quality of care concerns, barriers to care progression, and discharge delays. Formulate, coordinate, and implement individualized plans of care and discharge plans to ensure safe, timely, and effective transitions. Collaborate with physicians, nursing, social work, and other healthcare professionals to manage and support patient care plans. Perform variance tracking and outcome analysis, identifying trends and opportunities to assess the effectiveness of clinical pathways and care management processes. Perform other related duties as assigned.

Education:

Bachelor's degree in Nursing Master's degree in Nursing, Social Work, Healthcare Administration, or a related field preferred. Licensure and Certification Current, unrestricted RN license required. Case Management certification such as CCM or ACM preferred. Experience Minimum of three to five years of experience in inpatient care management, case management, utilization review, or a related acute care setting. Experience working within interdisciplinary healthcare teams required. Prior leadership or care coordination experience preferred. Knowledge, Skills, and Abilities Strong knowledge of care management principles, utilization management, discharge planning, and regulatory requirements. Ability to assess clinical, psychosocial, and resource needs across the continuum of care. Excellent communication, collaboration, and organizational skills. Demonstrated ability to analyze outcomes and identify opportunities for quality and process improvement. Proficiency with electronic medical records and care management systems. #LI-ES1
