About Us:
ONE Mobile Drug Testing (American Green Spring Diagnostics Inc.) is a premier, fast-growing occupational health and compliance clinic located in the San Dimas industrial corridor. We provide rapid drug/alcohol screening, DNA testing, and OSHA compliance services to hundreds of local logistics, transportation, and manufacturing companies. We are expanding our clinic services to include in-house DOT Physicals and are looking for an energetic, FMCSA-certified professionals to join our team as our Chief Medical Examiner. 1. Services and Responsibilities As a Certified Medical Examiner, you will be responsible for: Conducting comprehensive, FMCSA-compliant DOT Physical Examinations for commercial motor vehicle (CMV) drivers. Determining driver medical fitness for duty in strict accordance with 49 CFR Part 391.41 and FMCSA guidelines. Accurately documenting exam results, issuing Medical Examiner's Certificates (Form
MCSA-5876
), and uploading results to the FMCSA National Registry and our internal clinic software (FormFox) in a timely manner. 2. Independent Contractor (1099) Status 3. Schedule and Location This is a part-time, as-needed engagement. Your schedule will be determined on a flexible basis, coordinated mutually between you and management based on clinic volume and your availability. Services may be rendered at our professional clinic located in San Dimas, CA, or via our mobile dispatch unit at local client warehouse/yard locations. You have the flexibility to accept or decline offered shifts, including weekend (Saturday) coverage or extended hour blocks as client demand requires. 4. Compensation For your professional services, you will be compensated at a rate of [$45.00 + per completed exam ] . Payments will be issued on a /monthly] basis following the submission of a contractor invoice. 5. Professional Requirements and Contingencies This offer of engagement is contingent upon the successful verification and continuous maintenance of the following credentials: An active, unrestricted California state license to practice as a [Nurse Practitioner / Physician Assistant]. Active certification and listing on the FMCSA National Registry of Certified Medical Examiners (NRCME). Proof of active professional liability/malpractice insurance covering your independent contractor duties. Completion of a standard background check and execution of a W-9 form.
