Documentation Specialist

Key Responsibilities Review clinical documentation (OASIS, visit notes, plans of care, recertifications) for completeness, accuracy, and compliance with Medicare and agency standards Identify documentation discrepancies and coordinate corrections with clinicians in a timely manner Track, manage, and follow up on physician orders to ensure signatures and compliance requirements are met Maintain organized and up-to-date patient medical records in the EMR system Assist with audit preparation and ensure documentation meets regulatory guidelines (CMS, Medicare, and accrediting bodies) Communicate effectively with nurses, therapists, physicians, and administrative staff to resolve documentation issues Support HR functions, including onboarding documentation, staff file maintenance, and compliance tracking Assist with billing processes for long-term care patients, ensuring documentation supports claims submission Monitor documentation timelines to prevent delays in billing and reimbursement Maintain confidentiality and compliance with HIPAA regulations
