Care Manager Supervisor Huntingdon Office Huntingdon, PA 1 DAYS AGO 22708666 Summary Huntingdon, PA In-Person Competitive Salary 3 Years Experience Bachelor's degree No Commission 37.50 hours per week / Day Shift / Full-Time Description We're seeking a compassionate, organized, and motivated Care Management Supervisor to lead key service programs and support our dedicated care management team. In this role, you'll ensure that individuals in our community receive high-quality, efficient, and timely services—while guiding staff, improving processes, and strengthening partnerships across the human services network. What You'll Do
  • Supervise staff and daily operations in the Huntingdon County office
  • Oversee agency programs including Options (personal care, attendant care, home support, personal assistance), Protective Services, Guardianship, Caregiver Support, Legal Services, and others
  • Ensure all referrals are addressed within required timeframes
  • Supervise the Information & Referral Case Aide and verify accuracy and quality of their work
  • Review and collect monthly staff reports and monitor program performance
  • Review legal service applications in collaboration with the Service Manager
  • Represent the agency at Human Service Council meetings and share service-related updates
  • Review staff training needs and support ongoing professional development
  • Approve timecards, ensure accuracy of reported hours, and complete employee evaluations
  • Assist with office coverage as needed, including information and referral calls
  • Stay knowledgeable about local resources and eligibility criteria to help consumers access programs such as PACE, Property Tax/Rent Rebate, fuel assistance, SNAP benefits, and other supports What You Bring
  • Bachelor's degree and at least 3 years of experience in public or private social work - OR - An equivalent combination of experience and education, including 12 semester hours in sociology, social welfare, psychology, gerontology, or related social sciences
  • Strong leadership, communication, and organizational skills
  • A commitment to serving older adults, individuals with disabilities, and people in need of support
