The Family Partner will join our Community Transition Team (CTT) that is located in Holyoke, MA. About the
Role:
The Family Partner plays a vital role in assisting the Community Transition Team (CTT) by helping them understand individual family dynamics, strengths, and necessary supports. In this role, you will empower families during youth transitions home by identifying needs, facilitating service connections, and linking parents to valuable community-based resources—either to supplement or serve as an alternative to CTT services.
Key Responsibilities:
Support the Community Transition Team in identifying family strengths and needed resources. Help families successfully navigate the youth's transition back home. Identify, facilitate, and connect parents/guardians to community-based resources and support systems. Build strong, supportive relationships with families based on shared or lived understanding.
Requirements:
Education:
High School Diploma or equivalent.
Lived Experience:
Personal experience navigating state systems of care as a parent or guardian (preferably with the Department of Youth Services / DYS).
Driver's License:
Valid driver's license.
Transportation:
Reliable vehicle for work use (mileage reimbursement provided) .
Preferred Qualifications:
Preference given to candidates who have a child or relative who has aged out of DYS. Prior experience with the MA Department of Children and Families (DCF) or court-related involvement in the juvenile justice system is considered a plus.
Pay Rate:
$20.00 / hour
Schedule:
Monday-Friday, 9:00 AM-5:00 PM (includes one evening per week until 7:00 PM).
