Job Description

Tender Touch Rehab Services seeks a compassionate clinician for Dawnview, a skilled nursing and long-term care community. In this role, you will assess geriatric and sub-acute residents, design individualized treatment plans, and deliver high-quality rehabilitation to improve function, safety, and quality of life. You'll work closely with an interdisciplinary team in a collaborative, patient-focused culture that values integrity, open communication, and clinical excellence. Responsibilities Evaluate geriatric and sub-acute residents and develop individualized treatment plans Deliver evidence-based rehabilitation interventions to improve function and quality of life Collaborate with nursing, medical, and therapy teams on coordinated care Track progress, document treatments, and update care plans per facility and regulatory standards Educate residents and families on exercises, safety, and carryover strategies Participate in interdisciplinary meetings and quality improvement initiatives Maintain compliance with company, state, and federal guidelines Contribute to a compassionate, patient-focused culture within the facility Required Skills Geriatric rehabilitation Skilled nursing facility rehab Functional mobility training ADL and self-care training Therapeutic exercise and neuromuscular re-education Care planning and documentation in EMRInterdisciplinary team collaboration Patient and family education Fall prevention and safety training Outcome measurement and reassessment
