  • There is an on-call rotation with this position.
It includes 1 night a week, and every 3-5th weekend. We expect unfailing honesty, respect for the dignity and individuality of human beings, and a commitment to professional services. Responsibilities:
Provides direct supervision and daily care of Stay Graceful clients primarily in their home environment. Implements and maintains a professional and caring relationship with all clients; providing reliable care, limit setting and boundaries while demonstrating understanding of the clients' needs at all times. Plans and participates in activities with the clients as outlined in the care plan. ? Attends and actively participates in weekly team meetings, group supervision, and completes in-service hours annually as required by state regulations. Maintains a professional, courteous and helpful relationship with clients family and other primary social supporters. Adheres to Stay Graceful's Standard Operations Procedures and South Dakota State Regulations/Administrative Rules relating to in-home care. Assures all clients, coworkers and family members are treated equally and with respect. ? Responsible for the quality of services and completion of necessary in-home care duties and related training including:
  • Medication Management
  • CPR training
  • First Aid training
  • Training in life skills and other care techniques and adherence to these techniques
  • Daily care support may include assisting clients in household cleaning, laundry, cooking meals, grocery shopping, communicating with family and friends, and personal grooming.
Completes and/or assures completion of necessary documentation and reports by the work day. Knowledgeable and reflect Stay Graceful's Mission Statement, Core Values and Vision Statement. Each In-Home Care Provider is expected to meet Stay Graceful's Code of Conduct, Professionalism, and Ethics. Other duties as assigned. On Call 1 night a week and every 3/4th weekend
Education and Experience Equivalencies:
No degree required. Applicable experience strongly preferred in Human Services or a related field. Applicants with excellent abilities in communication via email, Word, and medical records preferred. All candidates must have (or qualify to obtain) a current South Dakota driver's license, proof of insurance and be cleared through the Division of Criminal Investigation.
Physical Requirements:
Ability to perform duties as outlined above including lifting 10-20 lbs. Continual standing and walking during shift. Continual talking in person or on the phone during shift. Ability to perform duties as outlined above including lifting 10-20 lbs. Will occasionally encounter toxic chemicals during shift.
PLEASE NOTE
Applicants who have been charged with a DUI within the past 5 years or have ever been charged with a violent crime or felony are ineligible for employment with Stay Graceful.
