Job Description
Job Overview Heartwarming Care is seeking a dependable and flexible Lead Caregiver to provide direct client care while serving as a primary on-call responder for schedule gaps, caregiver call-offs, and urgent coverage needs. In this vital role, you will serve as a primary point of contact for clients and their families, providing exceptional care and support while ensuring safety, comfort, and well-being. Your energetic approach will help coordinate on-call services, assist with care plans, and respond promptly to client needs across various settings including home health, assisted living, and community care environments. This is a critical role within our agency, responsible for ensuring continuity of care for our clients. The ideal candidate is reliable, responsive, and able to work short-notice assignments when needed.

Work Expectations Must be available for on-call coverage as scheduled Must be willing to accept short-notice assignments Must maintain punctuality and reliability at all times Key Responsibilities Provide personal care, homemaker, and companionship services to clients Serve as on-call caregiver for call-offs, no-shows, and urgent coverage needs Respond to scheduling needs within 15-30 minutes Accept same-day and short-notice assignments Assist with maintaining schedule stability Accurately document services provided Communicate changes in client condition to the office Responsibilities Act as the on-call caregiver for clients, providing immediate support and assistance during emergencies or urgent situations Support clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility assistance using equipment such as Hoyer lifts or resident lifts Monitor patient health by observing vital signs, medication administration, and overall condition; document findings accurately in compliance with HIPAA regulations Assist with meal preparation, light cleaning, and maintaining a safe environment that adheres to infection control standards Ensure compliance with all safety protocols including CPR and first aid procedures; participate in ongoing training related to patient observation and emergency response Requirements Previous caregiving experience Reliable transportation Valid driver's license and vehicle insurance Strong communication and time management skills Flexible availability (including evenings and weekends) Ability to respond quickly to urgent staffing needs Qualifications Proven experience in home & community care settings such as assisted living facilities, nursing homes, group homes or hospice environments Valid certifications in CPR and first aid; additional certifications in HIPAA compliance or nursing are advantageous Experience working with individuals with disabilities or developmental challenges; familiarity with assistive devices like Hoyer lifts or resident lifts preferred Ability to lift heavy objects safely and perform physically demanding tasks required for patient transfer and mobility assistance Excellent communication skills to effectively coordinate with clients' families and healthcare teams Compassionate attitude combined with a proactive approach to delivering high-quality caregiving support Join us in creating a caring environment where every individual receives personalized attention that promotes dignity, independence, and well-being. Pay:
