Up to $60/hour Job Summary Pride Health is seeking an experienced Licensed Master Social Worker (LMSW) for a contract opportunity at a leading acute care hospital in New York, NY. This role involves providing psychosocial assessments, supportive therapy, discharge planning, and care coordination for patients in an inpatient hospital setting. Responsibilities- Conduct psychosocial assessments for patients and families
- Provide supportive counseling and therapeutic interventions
- Coordinate discharge planning and community referrals
- Collaborate with interdisciplinary healthcare teams to support patient care goals
- Assist patients with care transitions and continuity of care planning
- Maintain accurate and timely documentation of patient interactions and care plans
- Advocate for patient needs and appropriate healthcare resources
- Support patients in inpatient medicine, surgery, and rehabilitation settings Requirements
- Master's Degree in Social Work required
- Active NYS Licensed Master Social Worker (LMSW) license required
- Minimum 1 year of LMSW experience required
- Hospital or acute care experience strongly preferred
- Experience in med/surg or rehab settings preferred
- Strong communication, assessment, and care coordination skills
