Up to $60/hour Job Summary Pride Health is seeking an experienced Licensed Master Social Worker (LMSW) for a contract opportunity at a leading acute care hospital in New York, NY. This role involves providing psychosocial assessments, supportive therapy, discharge planning, and care coordination for patients in an inpatient hospital setting. Responsibilities

• Conduct psychosocial assessments for patients and families
• Provide supportive counseling and therapeutic interventions
• Coordinate discharge planning and community referrals
• Collaborate with interdisciplinary healthcare teams to support patient care goals
• Assist patients with care transitions and continuity of care planning
• Maintain accurate and timely documentation of patient interactions and care plans
• Advocate for patient needs and appropriate healthcare resources
• Support patients in inpatient medicine, surgery, and rehabilitation settings Requirements
• Master's Degree in Social Work required
• Active NYS Licensed Master Social Worker (LMSW) license required
• Minimum 1 year of LMSW experience required
• Hospital or acute care experience strongly preferred
• Experience in med/surg or rehab settings preferred
• Strong communication, assessment, and care coordination skills
