Job Description

Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The LPN will provide high-quality, patient-centered care across various clinical settings, including long-term care facilities, hospitals, outpatient clinics, and home health environments. This role involves performing comprehensive patient assessments, administering medications and treatments, and collaborating with interdisciplinary teams to ensure optimal health outcomes. The ideal candidate will possess strong clinical skills, excellent communication abilities, and a commitment to delivering respectful and effective care to diverse patient populations. Responsibilities Conduct thorough patient assessments, including vital signs, physical examinations, and health histories, utilizing physiology knowledge and medical terminology. Administer medications via injections, IV infusions, catheterization, and other methods following aseptic techniques and sterile processing protocols. Monitor patients' conditions closely, managing acute pain, airway management, ventilator support, and responding promptly to emergent situations such as trauma or infectious disease outbreaks. Document all patient interactions accurately within electronic health records (EHR) systems like Epic or Cerner while ensuring compliance with HIPAA regulations. Assist with diagnostic evaluations such as laboratory specimen collection/processing, medical imaging procedures like sonography or radiography, and perform basic life support during emergencies. Provide specialized care for populations including geriatrics with dementia or Alzheimer's disease, pediatric patients in neonatal or pediatric units, and individuals with developmental disabilities or requiring hospice care. Support discharge planning and case management activities to facilitate smooth transitions between care settings. Maintain infection control standards and ensure adherence to NCQA standards for quality assurance. Collaborate effectively with physicians, therapists, social workers, and other healthcare professionals to deliver comprehensive patient care. Experience Valid Licensed Practical Nurse license in accordance with state regulations. Prior experience working in hospital medicine units such as ICU (Intensive Care Unit), Level I Trauma Center settings, or emergency rooms is highly desirable. Demonstrated proficiency in EMR/EHR systems including Epic, Cerner, or Athenahealth. Experience with medical-surgical nursing, acute care environments, or post-acute care settings such as skilled nursing facilities or assisted living communities. Knowledge of ICD coding (ICD-9/ICD-10), CPT coding for procedures like suturing or spinal taps is advantageous. Background in managing complex cases involving pain management, infectious diseases, or chronic conditions such as diabetes or cardiovascular issues. Exposure to specialized areas such as ophthalmology procedures (e.g., dermal fillers), dialysis units, or home & community care is a plus. Strong understanding of anatomy and physiology principles relevant to patient assessment and treatment planning. Ability to work effectively with diverse populations including children (toddler care), infants (neonatal ventilator), and older adults requiring memory or behavioral health support.
