In this vital role, you will provide high-quality, person-centered care across various settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. Your expertise will support patient recovery, comfort, and safety while working collaboratively with multidisciplinary teams. Duties Perform comprehensive patient assessments by monitoring vital signs, conducting physical examinations, and evaluating patient needs. Administer medications and injections accurately following physician orders while ensuring proper documentation in electronic health records (EHR) systems such as Epic or ALIS. Provide specialized care for patients with conditions like Alzheimer's disease, dementia, or those requiring memory or hospice care. Collaborate with physicians and other healthcare professionals to develop and implement personalized care plans for diverse patient populations. Educate patients and their families on health coaching, medication management, discharge planning, and post-acute care strategies to promote recovery and well-being. Experience Valid state licensure as a Licensed Practical Nurse with current CPR/BLS certification. Demonstrated proficiency with EMR/EHR systems. Prior experience in critical care environments such as ICU or PICU is highly desirable. Familiarity with

ICD-9/ICD-10

coding. Experience providing specialized care in areas like infectious disease management, hospice services, or behavioral health settings is a plus. Knowledge of anatomy, physiology, pharmacology, and medical terminology necessary for comprehensive patient assessment and diagnostic evaluation.
