Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and detailed health histories. Administer medications and injections accurately, including IV infusions and other specialized treatments such as botulinum toxin therapy. Utilize EMR (Electronic Medical Records) systems like Epic, Cerner, or Athenahealth to document patient care activities precisely and efficiently. Assist with diagnostic evaluations such as laboratory specimen collection, processing, and medical imaging procedures. Manage airway support equipment including ventilators and perform tube feeding as necessary for patients with complex needs. Conduct wound care, catheterizations, suturing, and sterile procedures following aseptic techniques to prevent infections. Collaborate with interdisciplinary teams in acute care settings like ICU or trauma centers to coordinate patient discharge planning and case management. Provide specialized dementia and Alzheimer's care in memory or assisted living facilities while ensuring safety and comfort. Support patients with developmental disabilities or behavioral health needs through tailored nursing interventions. Maintain compliance with HIPAA regulations by safeguarding patient confidentiality during all documentation and communication activities. Experience Valid licensure as a Licensed Practical Nurse (LPN) in the relevant state or jurisdiction. Demonstrated experience working in diverse healthcare environments such as hospitals (including Level I Trauma Centers), nursing homes, hospice settings, or outpatient clinics. Familiarity with EMR/EHR systems such as Epic, Cerner, eClinicalWorks, or Athenahealth for accurate documentation. Hands-on experience with IV insertion, phlebotomy, medication administration, and infection control protocols. Experience working with individuals with disabilities or those requiring complex chronic disease management like diabetes or infectious diseases. Strong understanding of anatomy, physiology, pharmacology, and medical terminology to support precise patient assessments and treatments.
