Videos Duties Help Duties include but are not limited to: Held accountable for delivering direct pient care from adults through geriric Veterans for both acute and chronic diseases in an outpient setting. Implement basic and advanced LPN procedures as defined by policies and procedures. Completion of pertinent clinical reminders and preventive health screenings. Prepare and maintain exam/trement rooms, including organizing supplies and equipment necessary for pient care. Assist with minor clinical procedures and ensure safety and comfort of pients during procedures. Providing pient and family health educion or instruction, to include self-management, prevention and wellness. Document pient care activities accurely and in a timely manner using the electronic health record (EHR) system. Participe in team huddles and case conferences, providing input on pient care plans and follow-up stregies. Assist with pient scheduling and appointment follow-ups to optimize clinic workflow.

In accordance with 38 U.S.C. 7402(d), no person shall serve in direct pient care positions unless they are proficient in basic written and spoken English. Educion - Gradue of a school of practical or vocional nursing approved by the approprie Ste agency and/or accredited by the Nional League for Nursing Accrediting Commission (NLNAC) the time the program was completed by the applicant. Exceptions include health care educion in the military service or training in the military service which is accepted by the licensing body in the jurisdiction in which the individual is licensed as qualifying for full LPN/LVN licensure accepted as meeting the educion requirements for VHA employment OR granted a license by a jurisdiction th does not require graduion from an approved school AND have least one year of successful practice as an
LPN/ LVN.
Licensure - Active, full, current and unrestricted licensure as a Gradue Licensed Practical or Vocional Nurse in a Ste, Territory, or Commonwealth (i.e., Puerto Rico), of the United Stes or the District of Columbia.
Grade Determinions :
In addition to the basic requirements sted above, the following qualificion criteria must be met for each grade. The candide's qualificions must clearly demonstre the level of competence required for the grade. GS-03 None beyond the basic requirements. LPNs this grade level are expected to perform all duties within the scope of their license (e.g., medicion administrion); however, completed work may require routine or detailed higher level review depending upon the complexity of the duties involved. GS-04 (a) Six months of qualifying experience as an LPN or LVN; or(b) Graduion from an approved school (which may be waived as provided in paragraph 4 of Section C) and one year of experience th involved nursing care work in a hospital, outpient clinic, nursing home, or other supervised medical, nursing, or pient care facility th provided a practical knowledge of human body structure and sterile techniques and procedures; or (c) Graduion from an approved school of least 24 months durion. GS-05 Knowledge and ability to provide a full range of practical nursing care to pients with a variety of physical and/or behavioral problems. Ability to serve as a responsible member of a nursing team and interact in an approprie manner with pients, family members, professional and/or other support personnel involved in the delivery of pient care. Knowledge and skill sufficient to prepare, administer, and appropriely document actions taken specific to commonly prescribed oral, topical, subcutaneous, intramuscular and/or intravenous medicions. This includes observing and documenting pient responses to medicion and reporting noted changes in the pient's condition. Knowledge and ability to recognize the need for and to institute emergency measures and assist in resuscition procedures in cardia and/or pulmonary arrest. Recognize and appropriely respond to breakage/malfunction or loss of equipment, safety hazards, and supply deficiencies. GS-06 Proficiency in initiing, performing, and completing assigned duties in providing care to variable pient populions. Knowledge and ability to appropriely carry out assigned pient care based on the pients' conditions, using judgment in selecting the approprie order and sequence of procedures and trements, and accurely recognizing, reporting, and recording relevant pient informion Ability to observe, identify, and respond to a pient's needs for care including medicion, equipment-assisted care, and pient/family educion. In organizing and delivering care, the LPN/LVN recognizes and considers emotional, socio-economic, and age-reled factors. Prepares and administers prescribed medicions (oral, topical, subcutaneous, intramuscular and/or intravenous) and performs trements according to established policies/procedures. Observes for, documents, and reports physical and/or emotional changes in the pient's condition from prescribed medicions/trements. Knowledge and ability to recognize urgent or emergent pient care situions and initiing approprie emergency interventions as directed. Knowledge and understanding of human behavior, pient motivions and reactions to situions and the ability to appropriely utilize this knowledge in working effectively with pients, family members and other staff. Establishes constructive relionships with individual pients and their families to elicit feelings and titudes, to promote positive relionships, communicion, and socializion skills. Fosters an environment of respect for individual pient and family rights to privacy and dignity in all aspects of care delivery. Effectively incorpores knowledge and understanding of customer service standards in all interactions with pients, family members, and/or other internal/external customers. Knowledge and skill in performing support duties for complex diagnostic tests and/or specialized practices or procedures, which includes preparing the pient and assisting in the diagnostic examinion, preparing, and handling specialized instruments or other specialized equipment, and monitoring the pient's condition before, during, and following the procedure. Serves as a preceptor in orienting, educing, and training less experienced LPNs/LVNs or NAs/HTs reled to support duties for these more complex, specialized tests/procedures. Actively seeks out educional opportunities to enhance nursing knowledge and skills, sharing new knowledge gained with other staff to improve and advance nursing practice.

This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. The incumbent may be exposed to infected pients and contamined merials and may be required to don protective clothing in isolion situions or operive/invasive procedures. The incumbent may occasionally be exposed to pients who are combive secondary to delirium, dementia, or psychiric disorders.
