Job Description

Job Summary, Responsibilities and Duties We are looking for a full-time LPN to work the following area: Hamden, New Haven, West Haven, Milford, Waterbury and the Valley. Must have Home Care Experience as an LPN. The LPN will supplement the nursing care needs of the patient as designated by the Registered Nurse Case Manager in the plan of treatment, under the supervision of Clinical Services. Such care may include teaching, maintenance, health promotion, coordination of nursing care according to the patient plan of care. Each LPN is responsible for preparing clinical notes, and assisting the patient/family/caregiver in learning self care techniques. Qualifications and Skills
  • Minimum of two years medical/surgical nursing experience required-minimum of 2 years experience within a home health setting is preferred.
  • Graduate of an accredited LPN School of Nursing
  • Connecticut LPN License
  • Ability to implement appropriate components of the nursing process and accept directional supervision from the primary nurse
  • Excellent physical assessment skills
  • Ability to teach patients,families,and/or caregivers which promotes recovery and independence
  • Ability to collaborate and communicate with peers,patients,families and physicians.
  • Organizational,problem solving and communication skills
  • Excellent oral and written communication skills
  • Ability to work independently and complete charting in a timely manner
Special Requirements:
  • Must have reliable transportation to commute to and from patient's homes.
  • Computer skills
  • Able to lift and move patients and carry necessary supplies
  • Manual dexterity and ability to climb and descend stairs
