Job Description
Overview
We are seeking a dedicated and compassionate LPN to join our dynamic healthcare team. In this vital role, you will provide expert pediatric care in patient's homes ensuring safety, comfort, and optimal health outcomes. Your energetic approach and attention to detail will help foster a positive environment for patients, families, and colleagues alike. Responsibilities
Conduct comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform individualized care plans.
Administer medications and injections accurately, including IV infusions and medication management, following strict aseptic techniques.
Perform specialized procedures such as suturing, catheterization, tube feeding, and ostomy care with precision and adherence to safety protocols.
Utilize electronic health record (EHR) systems to document medical records, treatment plans, and patient progress thoroughly and accurately.
Collaborate with patient's healthcare team.
Maintain compliance with HIPAA regulations and infection control standards while delivering exceptional patient-centered care across diverse populations including pediatrics, geriatrics, behavioral health, or individuals with developmental disabilities.
Requirements
Valid LPN license in good standing with relevant state nursing board.
Demonstrated proficiency with EMR/EHR systems.
Strong clinical skills including IV insertion, phlebotomy, aseptic technique, ventilator management, and pain management techniques.
Knowledge of anatomy and physiology combined with excellent patient assessment capabilities.
Ability to work effectively in high-pressure situations.
Experience working with diverse populations including long-term care residents, hospice patients, or individuals with disabilities is desirable.
Certifications such as Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS), or Pediatric Advanced Life Support (PALS).
