Licensed Practical Nurse

Job Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. In this vital role, you will provide nursing support, medication administration, resident monitoring, and coordination of care for residents in the this assisted living setting. The LPN will work collaboratively with resident assistants, providers, families, pharmacies, and facility leadership to support the health and well-being of residents in a non-medical assisted living environment. Responsibilities Observe, assess, and report changes in resident condition Monitor Vital signs and general wellness as per physician order and as needed Communicate resident concerns to Clinical Director, physician, and families Respond appropriately to emergencies and changes in condition Assist with infection prevention and control practices Administer medications and treatments as permitted under Tennessee law and facility policy Monitor for medication side effects and adverse reactions Provide guidance and support to resident assistants Maintain accurate and timely documentation Maintain confidentiality in accordance with HIPAA standards Requirements Current Tennessee LPN license in good standing Current BLS certification preferred Experience in assisted living, long term care, geriatrics preferred but not required Ability to communicate effectively with residents, families, staff, and providers Ability to work independently and as part of a team Ability to assist residents with mobility
