Role:
CenClear's Behavioral Health Division is seeking a compassionate and committed Licensed Practical Nurse (LPN) to join our team. What You'll Do:
Work 40 hours per week, 52 weeks per year Current schedule is four 10-hour days with Mondays off (subject to change based on program needs) Occasional travel required for coverage within a one-hour proximity. Assist psychiatric prescribers in providing treatment to clients, collecting labs, reports, information, and vitals for provider Follow up with clients per prescriber request Willingness to learn and adapt to new information and skills and engage in ongoing education, training, and meetings. Self-motivation and ability to work independently as well as a team. Empathy and understanding in dealing with clients and families. Update client history and changes at each appointment. Maintain professionalism and confidentiality Complete paperwork accurately and in a timely manner. Attend and participate in monthly supervisions. Participate in all required CenClear, CCBHC and wellness/health related education and trainings Provide wellness coaching and related training to staff, individuals in services and community partners Promote a culture of health and wellness through ongoing participation in meetings and community outreach Collaborate and coordinate with healthcare providers for continuity of care Facilitate referrals, as needed Collect required data for outcomes measurement and contract requirements and document appropriately Submit all necessary administrative paperwork according to procedures Document all wellness coaching sessions and other wellness/integration activities
What We're Looking For:
Licensed Practical Nurse (LPN) Two years of experience in mental health services Maintain professional and confidential attitude. Strong communication and interpersonal skills. Attention to detail and accuracy in documentation. Flexibility to travel and work varied hours. Ability to gather and analyze health-related information. Must obtain the following clearances prior to hire :
PA State Police Criminal History, PA Child Abuse History, FBI Criminal History, and Mandated Reporting Certificate Perks You'll Love:
