Job Description

  • LOOKING
FOR 11PM-7AM IMMEDIATELY
  • Patient Assessment and Care Planning:
  • Conduct thorough assessments of residents' health status, including physical, mental, and emotional well-being.
  • Develop individualized care plans in collaboration with the interdisciplinary team, considering residents' unique needs and preferences.
Medication Administration:
  • Administer medications as prescribed by physicians, ensuring accuracy and compliance with established protocols.
Monitor residents for any adverse reactions and promptly report any concerns to the appropriate healthcare professionals.
Wound Care and Treatment:
  • Perform wound care procedures, including cleaning, dressing changes, and monitoring healing progress.
Implement preventive measures to minimize the risk of pressure ulcers and other skin conditions among residents.
Monitoring Vital Signs and Health Parameters:
  • Regularly assess and document residents' vital signs, such as blood pressure, pulse, temperature, and respiratory rate.
  • Monitor other health parameters, such as blood glucose levels, oxygen saturation, and fluid intake/output. Assistance with Activities of Daily Living (ADLs):
  • Provide assistance to residents with various ADLs, including bathing, grooming, dressing, toileting, and mobility.
  • Encourage independence while offering support as needed.
Health Education and Counseling:
  • Educate residents and their families about their health conditions, treatment plans, and preventive measures.
  • Provide counseling and emotional support to residents facing health challenges or adjusting to life in a long-term care setting.
Collaboration with
Interdisciplinary Team:
  • Work collaboratively with physicians, nurse practitioners, therapists, social workers, and other healthcare professionals to ensure coordinated and holistic care for residents.
  • Participate in care conferences and contribute insights to the development and modification of care plans.
Documentation and Record-Keeping:
  • Maintain accurate and up-to-date documentation of all nursing assessments, interventions, and observations.
  • Ensure compliance with regulatory requirements and facility policies regarding documentation and record-keeping.
Emergency Response and Crisis Management:
  • Respond promptly and effectively to medical emergencies and crisis situations within the facility.
  • Administer first aid, initiate appropriate interventions, and coordinate with emergency medical services as needed.
Quality Improvement Initiatives:
  • Participate in quality improvement initiatives aimed at enhancing the quality of care and resident outcomes within the facility.
  • Identify opportunities for improvement and contribute to the implementation of best practices and evidence-based interventions.
SHARED TASKS
(Tasks that can be performed by all staff)
  • Serving food.
  • Providing and assisting impromptu activities.
  • Transporting residents within the home.
  • Light household duties.
  • These duties include but are not limited to: vacuuming, loading and unloading the dishwasher, dusting, wiping down kitchen and bathroom counters and sinks.
  • Dining assistant.
  • Responding to call lights and assist as needed.
  • Laundry - laundry at the household level.
  • Answering the telephone.
  • Answering the door.
  • Providing snacks and supplements (appropriate to resident's diet).
Pay:
$31.00 - $35.00 per hour
