Job Description

Position :
Licensed Practical Nurse Admissions/Treatment Shift/Schedule :
Monday-Friday 8am-4:30pm
  • Must have a valid nursing license in the state to practice
Duties and Responsibilities :
  • Respond to requests for medical record information by insurance companies, such as, Medicare, Medicaid, Veterans Administration (VA) or healthcare providers as authorized and in accordance with Health Insurance Portability and Accountability Act (HIPAA) privacy and security requirements.
  • Open, close, and ensure the proper completion, assembly, coding, signature, etc. for electronic health records (EHR)
  • Assist in interviewing residents, guardians, and sponsors to obtain required admission information and signatures.
  • Collaborate with internal and external sources to solve admission problems as needed in accordance with facility policies.
  • Collect, assemble and review admission documentation for accuracy and completion. Coordinate the pre-admission screening and approval process in accordance with facility policies and established best practices.
  • Forward completed medical records to the health information/medical records department as appropriate.
  • Maintain a current resident census; provide the census report to the Director of Nursing, Administrator and government personnel as necessary and in accordance with privacy rules. Maintain a resident waiting list in accordance with facility policies and procedures and as required by state/federal regulatory requirements.
  • Maintain various registries including registries for admissions and discharges of residents as directed. Notify nursing services of when the resident has arrived at the facility to ensure that a team member of the nursing staff escorts the resident to his/her assigned room.
  • Obtain the resident's assigned room number as well as signature on all necessary admission papers, permits, releases, etc.
  • Review and explain to the resident and/or representative room rates, billing procedures, visitor/guest privileges, restrictions, resident care procedures, etc.
  • Consult and coordinate with the interdisciplinary team (IDT) and healthcare professionals to plan, implement and evaluate individualized resident care plans.
  • Participate in completing sections of the Minimum Data Set (MDS) assessment as requested.
  • Participate in updating and reviewing policies and procedures related to skin and wound care.
  • Identify, manage, and treat primary and secondary wounds, including pressure ulcers/injuries, ostomies, surgical wounds, traumatic wounds and burns.
  • Help identify and mitigate risk factors for secondary wound infections.
  • Help identify and mitigate specific resident risk factors for pressure ulcers/injuries.
  • Assist the interdisciplinary team in identifying and managing resident pain related to wounds. Complete regular skin assessments on residents and document the findings.
  • Provide skin and wound care and treatments, as ordered.
  • Provide information and care instructions to residents and families related to their specific condition(s) and treatments(s) in a language and format that can be understood by the resident/family.
  • Evaluate pressure-relieving devices, therapies, dressings and wound care products and make recommendations to the interdisciplinary team regarding their efficacy and use.
  • Adhere to all facility infection prevention and control practices. Administer medications within the scope of practice and according to practitioner orders. Report adverse consequences, side effects or any medication errors.
  • Provide nursing care that is informed, compassionate and sensitive to residents with cognitive decline, memory loss or history of trauma.
  • Provide nursing care that is appropriate and sensitive to the culture, language and background of the resident.
Skills and Qualifications :
  • Graduate of an accredited school of nursing.
  • Valid registration as an LPN in the state of practice.
  • Completion of relevant State Board of Nursing approved.
  • Long-Term Care Experience
  • Ability to follow oral and written instructions.
  • Work as a team member independently and responsibly.
  • Document/Record observations and activities.
  • Possess good communication skills.
  • Proficient in word processing, email, and spreadsheet applications.
