Licensed Practical Nurse Arya at DeBary DeBary, FL Job Details Part-time | Full-time $28 an hour 1 day ago Qualifications CPR Certification BLS Certification CPR Assisting with ADLs Clinical documentation LPN Full Job Description Job Summary The Wellness Nurse plays a vital role in ensuring the health and wellbeing of residents at Providence Living. This position provides clinical oversight, leadership, and support to care staff, ensuring that resident care meets the highest standards of quality, safety, and compassion. Responsibilities include medication management, clinical assessments, regulatory compliance, staff supervision, and communication with residents, families, and interdisciplinary teams. Key Responsibilities Supervise Resident Assistants and delegate shift duties as needed. Provide coverage for call-outs to ensure continuity of care. Assist residents with activities of daily living (ADLs) such as bathing, dressing, toileting, hygiene, eating, and mobility per each Resident Service Plan. Administer medications in compliance with state regulations and company policies. Review medication and treatment documentation to ensure accuracy and completion. Monitor resident health, safety, and functional status, documenting changes on the 24-hour shift report. Assist in developing and updating individualized Resident Service Plans. Report significant changes in resident condition to the Director of Wellness in a timely manner. Coordinate emergency response for residents, including contacting EMS and preparing transfer documentation. Encourage resident participation in programs and activities; lead small group or one-on-one sessions when assigned. Ensure compliance with all federal, state, and local regulations, as well as company policies and standards. Complete required HIPAA, compliance, and regulatory training. Maintain confidentiality and uphold professional standards at all times.
Qualifications Education & Licensure:
Graduate of an accredited nursing program. Current, valid license as an LPN or RN in the state of Florida.
Experience:
Minimum 1 year of experience in assisted living, long-term care, hospice, home health, or a similar clinical setting preferred.
Certifications:
CPR Certification (required). Must meet state-specific requirements for medication administration, if applicable. Skills & Competencies Strong clinical judgment and nursing assessment skills. Excellent interpersonal and communication skills. Time management, multitasking, and organizational abilities. Ability to lead and coach care staff effectively. Proficient in Microsoft Office (Word, Excel, Outlook). Compassionate, resident-focused approach aligned with Providence Living's mission. Knowledge of HIPAA, assisted living regulations, and nursing best practices. Physical & Work Environment Requirements Regularly required to stand, walk, use hands, reach, talk, and hear. Must be able to lift, push, pull, or move up to 100 pounds . Frequent stooping, kneeling, crouching, and climbing stairs. Occasional weekend, evening, or night work required for coverage. Exposure to infectious diseases, bloodborne pathogens, latex, chemicals, and potential injury risks. On-call availability as needed.
