Job Description

Job Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. In this vital role, you will provide high-quality patient care across various settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. Your expertise will support patient recovery, comfort, and well-being through skilled nursing interventions. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems such as Epic or Cerner. Administer medications and injections accurately following physician orders while adhering to HIPAA regulations and aseptic techniques. Assist with complex procedures such as IV infusion, catheterization, tube feeding, and wound suturing to ensure optimal patient outcomes. Monitor patients' physiological status, manage acute pain, and provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments. Collaborate with multidisciplinary teams on discharge planning, case management, and coordination of post-acute or hospice care services. Conduct patient education on health coaching topics like medication management, nutrition, and chronic disease prevention. Maintain detailed medical documentation of patient care activities, specimen collection/processing, and medical records in compliance with NCQA standards. Qualifications Valid state licensure as a Licensed Practical Nurse (LPN) with current certification. Proven experience in critical care environments such as ICU or Level I Trauma Centers is highly desirable. Hands-on experience with EMR/EHR systems like Epic, Cerner, or Athenahealth for accurate medical documentation. Knowledge of
ICD-9/10
coding for diagnostic evaluation and billing purposes. Skilled in performing medical procedures including phlebotomy, venipuncture, sterile processing techniques, and airway management. Experience working with diverse patient populations including pediatrics, geriatrics, individuals with disabilities or developmental disabilities. Strong understanding of anatomy and physiology principles relevant to acute care settings such as emergency medicine or hospital medicine. Ability to work effectively in fast-paced environments like emergency rooms or urgent care clinics while demonstrating excellent patient assessment skills.
