Licensed Practical Nurse

Job Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The LPN will provide essential nursing care to patients across various settings, including long-term care facilities, hospitals, outpatient clinics, and home health environments. This role involves delivering direct patient care, assisting with medical procedures, and supporting the overall health and well-being of diverse patient populations. The ideal candidate will possess strong clinical skills, excellent communication abilities, and a commitment to delivering high-quality care in accordance with healthcare regulations and standards.

Responsibilities:

Provide comprehensive patient assessments by monitoring vital signs, evaluating symptoms, and documenting medical histories using electronic health record (EHR) systems such as Epic or Cerner. Administer medications and injections accurately, including IV infusions, ensuring adherence to safety protocols and HIPAA regulations. Assist with medical procedures such as catheterization, suturing, phlebotomy, and sterile processing techniques. Support patients with complex needs including dementia care, Alzheimer's care, memory care, and working with individuals with developmental disabilities or behavioral health conditions Manage airway management techniques and ventilator support for critical care patients in settings like ICU or Level I Trauma Centers. Conduct diagnostic evaluations such as physical examinations, specimen collection/processing, and basic imaging procedures like sonography or radiology assistance. Implement infection control measures and aseptic techniques to maintain a safe environment for patients and staff. Coordinate discharge planning, case management activities, and patient education to promote recovery and long-term wellness. Utilize EMR/EHR systems such as Athenahealth or eClinicalWorks for accurate medical documentation and patient records management. Participate in emergency response situations including triage in urgent care or emergency room settings; demonstrate proficiency in basic life support (BLS) protocols. Support specialized areas such as hospice care, palliative services, pediatric or neonatal units, and post-acute or sub-acute care environments.

MUST HAVE
