Director of Nursing Primary Responsibilities:
The Community Nurse is dedicated to providing the highest quality of care in a loving and safe environment for all of our residents, while helping each of them to achieve their maximum potential. We are committed to be a resource for the families of our residents who have entrusted us with their care. We will empower our staff by providing them with a motivating and supportive work environment.
ESSENTIAL JOB FUNCTIONS
1) Knowledgeable of Illinois Supportive Living Program Regulations and compliance needs. 2) Administer prescribed medications; treatment and/or emergency care as needed. 3) Document resident condition and any other pertinent information appropriately in the resident chart. 4) Assist in the preparation for annual licensure by HFS. 5) Observe ethics and confidentiality regarding resident rights. 6) Assist in ensuring nursing staff are providing care and services in accordance with the Illinois Nurse Practice Act. 7) Ensure resident annual assessments and plans associated with annual assessments are completed and in compliance with HFS supportive living regulations. 8) Ensure that all changes in conditions or outside providers are updated on resident's quarterly assessments and service plans. 9) Work with
DON/ADON
in tandem to maintain communication with residents managed care organizations. 10) Assist with coaching and embrace continued communication with nursing department. 11) Assist with quality assurance measures in accordance with supportive living regulations, regarding incidents/accidents, falls, infection control measures, etc. 12) Provide communication to in-house and outside physicians groups and outside providers to ensure up to date service planning for residents 13) Provide assistance with family or resident designee concerns in tandem with DON and nursing staff. 14)
RAI/ISP/ISS
- completion with co-signature of Registered Nurse. 15) Weekly updates (scheduled) with in house physicians' group, managed care providers, and outside providers. 16) Assistance with DON for monthly quality assurance measures. 17) Assisting with nursing department day to day needs. 18) Other duties as assigned.
Qualifications and Skills Required:
1) Must be a Licensed Practical Nurse or Registered Nurse in the State of Illinois 2) Have experience and knowledge of Assisted Living/Supportive Living Regulations and compliance needs. 3) Able to maintain regular attendance. 4) Self-directed, demonstrating initiative and follow-through 5) Must be inspired to serve the needs of residents. 6) Ability to multitask. 7) Excellent interpersonal, organizational and communication skills. 8) Must have the ability to read, write and follow written direction in the English language. 9) Ability to timely and effectively follow up on all necessary requests from residents and families. 10) Maintain and encourage a positive work atmosphere by communicating in a manner that fosters cooperation. 11) Proficiency in computer skills, Microsoft Office (Windows, Outlook, and Excel) with the ability to learn new applications. 12) Ability to handle stressful situations and work well under pressure.
