Job Description

Job Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. In this vital role, you will provide high-quality patient care across diverse settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. Your expertise will support patient recovery, comfort, and well-being through comprehensive assessments, medication administration, and collaborative care planning. Duties Perform thorough patient assessments, including vital signs, physical examinations, and health histories to inform care plans. Administer medications and injections accurately following physician orders while monitoring for adverse reactions. Assist with invasive procedures such as IV infusion, catheterization, and sterile dressing changes using aseptic technique. Document patient information meticulously in electronic health record (EHR) systems like Epic or Cerner to ensure seamless communication among care teams. Provide specialized care for individuals with complex needs such as dementia, Alzheimer's disease, or those requiring hospice or palliative services. Support emergency interventions including airway management, ventilator operation, and acute pain management in critical situations. Collaborate with physicians, case managers, and other healthcare professionals to develop and implement personalized care plans that promote recovery and independence. Experience Valid Licensed Practical Nurse license in accordance with state regulations. Proven experience working in diverse clinical environments such as hospital medicine, medical-surgical units, ICU or trauma centers, emergency rooms, or outpatient clinics. Critical care experience including Level I or Level II trauma center exposure is highly desirable. Familiarity with EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks for accurate medical documentation. Hands-on experience with IV insertion, phlebotomy, medication administration (including injections), and specimen collection/processing. Knowledge of medical terminology, anatomy and physiology to perform accurate patient assessments and diagnostic evaluations. Experience working with special populations such as pediatrics, neonatal ventilator management, or individuals with developmental disabilities is advantageous. Additional skills in infection control practices, HIPAA compliance, ICD coding (ICD-9/10), CPT coding for procedures, and discharge planning are preferred.
