Job Description

Role and Responsibilities Responsibilities:
Deliver nursing care in accordance with the plan of care and physician's orders. Monitor patient health, vital signs, and report changes to the RN or physician. Administer medications, treatments, and wound care. Educate patients and families on disease management and self-care techniques pertinent to diagnosis and the care plan. Assist with daily living activities such as bathing, dressing, and mobility. Document patient care accurately and timely using electronic health records. Collaborate with the healthcare team to coordinate comprehensive care and appropriately report identified needs for other disciplines to the registered nurse. Participate in care planning and discharge coordination. Use equipment and supplies effectively and efficiently. Maintain appropriate and applicable infection control standards including hand hygiene. Maintain a safe and clean working environment in compliance with healthcare procedures and regulations. Maintain compliance with Indiana Nurse Practice Act and agency policies. Maintain confidentiality of client information in compliance with HIPAA and company policies. Participate in professional growth, development, trainings, education and meetings as requested. Perform other duties as assigned by DON, RNCM or Administrator. Qualifications and Education Requirements Registered nurse (RN) or Licensed practical nurse (LPN) in the state of Indiana in good standing. One (1) year experience in nursing (preferred). Valid driver's license and good driving record: Required Preferred Skills Graduate of an accredited LPN program: required. Current and valid Indiana LPN license: required. Current CPR certification, applicable TB evaluation/testing, and valid driver's license: required Experience in home health or clinical care: preferred. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills, critical-thinking skills, interpersonal skills and nursing skills per competency checklist. Strong communication, organizational, and critical-thinking skills. Complies with accepted professional standards and practice. Ability to multi-task, work independently, and communicate efficiently. Experience in MS Office including Outlook and Word as well as general computer knowledge.
Position Physical Demands:
Prolonged or considerable walking or standing. Able to lift, position or transfer patients. Able to lift supplies and equipment, typically not expected to exceed 25 pounds in weight. Considerable reaching, stooping, bending, kneeling or crouching. Visual acuity and hearing to perform required nursing skills. Limited time at a computer or tablet screen charting. Limited to extended time driving within the state of Indiana.
