We are currently looking for a full-time Licensed Practical Nurse (LPN) for home care department.

Responsibilities Include:

Implement plan of care providing therapeutic nursing care and education to attain maximum patient and family independence and participation. Evaluate patient progress towards goals and confer with Case Manager regarding any potential changes in care prior to implementation. Assess goal achievement and service benefit to patients/families and consult with Case Manager regarding findings. Demonstrate flexibility in caring for patients with diverse physical and psychological needs. Record and document patient care services.

Qualifications:

Current LPN licensure in

MA. 1 - 2

years acute care experience required. Home care experience preferred. CPR certification. Valid driver's license and available transportation. Strong assessment and communication skills. Ability to build rapport with patients. Ability to handle physical workload, Work with a friendly and supportive team of seasoned clinicians at an agency that understands the need for work/life balance.
