Locations Available Acton, MA Arlington, MA Ayer, MA Concord, MA Groton, MA Lexington, MA Littleton, MA Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) specializing in Home Health Care to provide exceptional nursing services to patients in their homes. The ideal candidate will possess a strong clinical background, excellent communication skills, and a commitment to delivering personalized patient care. This role offers an opportunity to make a meaningful difference in the lives of individuals requiring ongoing health support, particularly within geriatric, developmental disabilities, and chronic illness populations. The LPN will work closely with interdisciplinary teams to ensure safe, effective, and compassionate care delivery while adhering to all regulatory standards. Responsibilities Conduct comprehensive patient assessments, including vital signs, physical examinations, and medication reconciliation, ensuring accurate documentation within electronic health record (EHR) systems. Administer medications, including injections and IV infusions, following prescribed protocols and maintaining strict adherence to HIPAA and clinical confidentiality policies. Provide wound care, catheterization, tube feeding management, and other skilled nursing interventions tailored to individual patient needs. Collaborate with physicians, case managers, and caregivers to develop and implement personalized care plans that address acute pain management, dementia care—including Alzheimer's and memory care—and chronic disease management. Educate patients and their families on health maintenance, medication administration, disease processes such as infectious diseases or respiratory conditions, and safety procedures. Monitor patient progress through regular assessments and adjust care strategies accordingly while maintaining detailed medical documentation. Ensure compliance with long-term care regulations and standards across various settings such as assisted living facilities, nursing homes, hospices, or home environments. Requirements Valid LPN license in good standing with current state licensure requirements. Proven experience working in home health care or related settings such as skilled nursing facilities or hospitals with exposure to medical-surgical, ICU, emergency medicine, or trauma center environments. Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, or Athenahealth for accurate documentation and patient record management. Strong clinical skills including vital signs monitoring, medication administration—including injections—and basic life support (BLS). Experience working with diverse patient populations including geriatric patients, individuals with developmental disabilities or dementia-related conditions such as Alzheimer's disease. Ability to perform specialized procedures such as venipuncture, sterile processing techniques, specimen collection/processing, and airway management when necessary. Excellent communication skills for effective caregiver communication and patient education; familiarity with case management practices is preferred.
