Job Description Shift:
This position is for 1st Shift, 2nd shift, 3rd shift . Candidates must be available during this time, with flexibility for holidays as needed and every-other weekends.

Purpose of Position The Licensed Practical Nurse (LPN) provides direct resident care and supervises nursing assistants in accordance with physician orders and care plans. Under the supervision of the RN Shift Supervisor, the LPN ensures quality, compassionate, and compliant nursing services are delivered. Key Responsibilities Provide direct nursing care and treatments as ordered by the physician Assist in developing and maintaining individualized resident care plans Administer and document medications accurately and safely Monitor and report changes in resident conditions to the RN Supervisor and physician Supervise and support nursing assistants in daily resident care Document resident care thoroughly and promptly Promote positive resident and family relationships through compassionate communication Ensure compliance with HIPAA, safety standards, and infection control procedures Participate in daily rounds and respond to emergencies as needed Qualifications Current, active Connecticut Licensed Practical Nurse (LPN) license in good standing Knowledge of nursing and medical procedures in long-term care Strong observational and reporting skills Ability to communicate effectively with residents, families, and staff Compassionate, patient, and team-oriented Experience in hospital or long-term care preferred, but not required Working Conditions & Requirements Frequent standing, walking, bending, and lifting throughout the shift Ability to push, pull, lift, and assist residents as required Flexibility to work weekends, holidays, and additional hours as needed Exposure to infectious waste, odors, and chemicals Benefits We are proud to offer competitive pay and a comprehensive benefits package, which may include: Health, dental, and vision insurance Paid time off (PTO) and holidays 401(k) retirement savings plan Life insurance Ongoing training and career development opportunities Join Our Team At Reading Center for Health and Rehabilitation , you won't just care for residents — you'll be part of a legacy of compassion, excellence, and community.
