Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) specializing in Long Term Care (LTC) to provide high-quality nursing care to residents in a skilled nursing or assisted living environment. The ideal candidate will possess a strong foundation in patient assessment, medication administration, and care planning, with a focus on promoting residents' health, safety, and well-being. This role offers an opportunity to work within a dynamic team committed to delivering personalized and respectful care to individuals with diverse needs, including those requiring memory care, hospice support, and complex medical management. About the Job Hiring 2 LTC LPNs in Sister Bay, WI 40 hours per week AM Shift every weekend, 3 PM shifts during week E/O weekend required 8 Weeks in length Onsite housing options available! BLS required Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation. Administer medications accurately while adhering to HIPAA regulations and sterile technique protocols. Monitor residents' conditions closely, managing acute pain, infectious diseases, or chronic illnesses with appropriate interventions. Assist with activities of daily living (ADLs). Collaborate with interdisciplinary teams to develop and update individualized care plans. Maintain meticulous medical documentation of treatments, medication administration, and patient progress in compliance with healthcare standards. Support discharge planning and post-acute care transitions by coordinating with families and community resources.
